NHS England is introducing the Summary Care Record which will be used in emergency care to improve the safety and quality of care received by providing better access to important and relevant information about your health.
Only three areas at this time have been agreed to be included in this summary record and they are:
This means if you have an accident or become ill the doctors treating you will have immediate access to this information about your health. The information is only available to authorised healthcare professionals and they will only access this information if they have your express permission to do so.
The practice will be participating in this project, unless you have instructed us otherwise, from March 31st 2015 a summary care record will be created for you.
You have a choice whether or not to have a Summary Care Record. If you wish to be included you need do nothing, should you wish to opt out then you would need to complete an opt out form, available to down load here, printed from the website www.nhscarerecords.nhs.uk, requested from the dedicated NHS Summary Care Record information line on 0845 603 8540 or obtain a copy from reception. Copies can be obtained in easy read format and other languages. Once completed please return it to the surgery. If you have previously completed this form you do not need to do so again.
